Following an investigation into an incident in Area 2 (Whangarei-Kaipara) the following safety consideration should be followed –
To ensure the safety of our people, where possible, staff should have two personnel present on cordons.
A recent incident occurred where a tirfor winch was being used at an incident.
Recently at a structure fire a motorist ignored the instructions of the sole Operational Support Officer (OSU) who was in charge of the cordon and removed a cone so he could drive through.
Recently a crew operated an appliance bay door without realising a painter was working off a ladder in the vicinity of the door. This event had the potential to cause injury to the contractor.
